Click on the survey link to participate in a study that examines the effects of health on mobility among Canadians.

Its goal is to determine how health as well physical, cognitive and social factors influence injury rates in an aging population.

PLEASE NOTE: The survey is completely confidential. Your name and any other identifying information will be removed from the database prior to analysis. All findings from the survey will be reported anonymously and secured on password-protected computers at the University of  Waterloo.
